Note: if the Land Title is not under the business permit applicant’s name, submit a notarized deed of sale/contract to sell/ consent ofuse from the property owner allowing the business to operate in the premises (2 photocopies)

1.2 Secure necessary clearances/ certifications/ permits from endorsing offices particularly Zoning Clearance, Building Official Clearance, Sanitary Permit, CENRO Clearance, Vet. Clearance (if the business is animal facility/ clinic, animal farming, selling of veterinary products), Tourism Clearance (businesses related to tourism)


1.3 Update records and print the e-business applications form upon
completeness of the clearances/certifications/permits from endorsing

3. Pay & Claim at Windows 8 or 9

3.1 (Pay the amount indicated in the Tax Order of Payment,and BFP Order of Payment Slip) 

Shall sign the Business Releasing Logbook and receive the Official Receipts, Business Permit, Sanitary Permit, Fire Safety Inspection Certificate, CENRO Clearance, Vet. Clearance (if the business is animal facility/clinic, animal farming, selling of veterinary products), Tourism Clearance (businesses related to tourism).

1.1 Shall receive payment of taxes, fees and charges.


1.2 Shall process and print Fire Safety Inspection Certificate (FSIC)


1.3 Shall approve and print Business Permit Certificate


1.4 Segregate the attached documentary requirements for Business tax payers and for office files.


1.5 Shall bring the Business Permit to the Local Chief Executive or authorized representative for approval and signature


1.6 Stamp a dry seal


1.7 Record in the releasing log book and have it signed by the client as
proof of receipt. 


1.8 Release the approved Business Permit, FSIC, Official Receipt, and other related clearances/ certificates, and Permits
