Unified Application Form for Business Permit (1 original copy) Business Permit and Licensing Office
Letter request signifying the intent for change of business name (1 original Copy) Business Owner
Valid Identification Card (1 photocopy) Business Owner
Business Permit, Sanitary Permit, FSIC, CENRO Clearance, Vet Clearance, Tourism Clearance (1 original copy each) issued in the current year Business Owner
DTI Business Name Registration (2 photocopies) Department of Trade and Industry
Authorization Letter, if applicant is an employee of the business establishment (1 original copy & 1 photocopy)
Employees Identification Card as proof of employment (2 photocopies)

1. Filing of Application for change of Business Name

1.1 Shall check for the completeness of the documentary
requirements submitted


1.2 Inform endorsing office/s concerned on the changes made
and secure updated clearance/s, certificate/s, Sanitary Permit, if applicable

2. Pay & Claim at BPLO Windows 8 or 9

2.1 Pay the amount indicated in the Order of Payment,

2.2 Shall sign the Business Releasing Logbook

2.3 Receive the Official Receipts, updated Business Permit, Sanitary Permit, Fire Safety Inspection Certificate, CENRO
Clearance, Vet. Clearance (if the business is animal facility/ clinic, animal farming, selling of veterinary products), Tourism Clearance (businesses related to tourism), etc

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

2.1 Shall receive payment for the change of business name.
2.2 Shall process the approved requested updates at Fire          Safety Inspection Certificate (FSIC)


2.3 Shall process the approved requested changes on Business
Name and print the updated Business Permit Certificate


2.4 Segregate the attached documentary requirements for Business tax payers and for office files.


2.5 Shall bring the Business Permit to the Local Chief Executive or authorized representative for approval and signature


2.6 Stamp a dry seal


2.7 Record in the releasing log book and have it signed by the client as proof of receipt. 

 

2.8 Release the approved Business Permit, FSIC, Official Receipt, and other related clearances/ certificates, and Permits.
