Post Activity Report (to be submitted 25 days after the Activity) (1 original copy)

Accomplishment Report Form (Gender Issues, GAD Mandates, GAD Objectives, GAD Activity, Performance Indicator and target, Actual Result, Actual Expenditure andVariance and Remarks

1.2.1 For Attribution – Title of the Program,
Actual Expenditure, HGDG Score, HGDG Attributed Amount and Variance and Remarks

1.2.2 (If there is a correction on the submitted documents, the personnel will ask for the soft copy to make a necessary Correction, thus, if the client didn’t have the soft copy, a note or correction will be made on the presented hard
copy)
