1. One (1) photocopy of the Journal of Public Hearing and one (1) photocopy of the attendance sheet of the public hearing conducted by the Sangguniang Barangay concern where the proposed land to be reclassified is located

2. One (1) photocopy of Resolution of the Sangguniang Barangay concern anent the intended reclassification

3. One (1) photocopy of BARC Chairman certificate as to the absence of tenants and pertinent tenancy claims on the land being sought to be reclassified

4. One (1) photocopy of Vicinity Map/Site Location Plan Signed/Sealed by a Licensed Geodetic Engineer

5. One (1) photocopy of Certified True Copy of TCT from the Registry of Deeds or Proof of ownership
