1.Present community tax certificate, fill out the request slip and verify with the staff of the City Veterinary Office for unclaimed animals’ candidates for adoption.	1.1. 2. The proposed owner shall affix his/her signature in the form and pay the pet registration fee.	2.1. Preparation of Animal Adoption Form in duplicate. 2.2. Prepare pet registration certificate and official receipt. 2.3. The City Veterinarian shall affix his signature in the Pet Adoption Form. A copy shall be issued to the proposed owner together with the pet registration certificate and official receipt, and another shall be kept as office file.	P50.00	10 minutes	Isagani C. 3. Proceed to Barangay Mate to claim chosen pet at the Animal Impounding Center by presenting the adoption form and pet registration certificate with official receipt, and prepare pet leash.	3.1.
