2. Receive and properly fill-up FOI Request Form |
2.1 Upon determination of the validity and legality of the of the purpose, the FOI Receiving Officer will ask the client to properly fill-up the FOI Request Form | None | 5 minutes |

3. Submit properly filled-up FOI Request Form together with a valid ID. |
3.1 The FOI Receiving Officer will review the submitted FOI Request Form. 3.2 If properly filled-up and the identity of the client has been established based on the valid ID presented, the transaction will proceed with the next step. 3.3 If improperly filled-up or the identity of the client cannot be established due to invalid ID presented, the FOI Receiving Officer will ask the client to properly fill-up the form and/or present additional valid ID for proper identification. Upon compliance, the transaction will proceed with the next step. |

4. Understand the Privacy Notice |
4.1 The FOI Receiving Officer will read and explain the legal provisions contained in the Privacy Notice. |

5. Wait for the Release of requested information or document (for simple request). |
5.1 The FOI Receiving Officer will process the requested document (for simple request or documents). 5.2 Or the FOI Receiving Officer will advise the client to wait for the notification on the availability of the document or will be provided to the client based on his preferred mode of reply (for highly technical or complex request). |

6. Receive the requested information or document, Sign in the logbook to acknowledge receipt of the requested document. |
6.1 Release the requested information or document on the client's preferred mode. Then record the transaction in the logbook. |
