ABOUT THE SERVICE:

Assistance for the proper accreditation process and procedure provided to POs, NGOs, CSOs and Other Organizations who are applying for a Sangguniang Panlungsod Accreditation Certificate.

CHECKLIST OF REQUIREMENTS:

CHECKLIST OF REQUIREMENTS WHERE TO SECURE
1. One (1) photocopy of duly accomplished and signed Application Form for Accreditation  1. Sanggunian Office, Records Officer desk 
2. One (1) photocopy Board Resolution 2. Applicant/Client supplied 
3. One (1) photocopy of Certificate of Registration issued by either SEC, DOLE, DTI, or CDA   3.1. SEC Headquarter, Salcedo Village, Makati City
  3.2. DOLE Headquarter, Intramuros, Manila 
  3.3 DTI Headquarter, Gil Puyat, Makati City
  3.4 CDA Headquarter, Quezon Ave, Diliman, Quezon City 
4. One (1) photocopy of document showing List of Current Officers and Members  4. Applicant/Client supplied
5. One (1) photocopy of document showing Original Sworn Statement stating that the organization is independent, non-partisan organization and that it will retain its autonomy while pursuing the advancement of the people’s interest through its membership in a local special body 5. 6. One (1) photocopy of document showing Annual Accomplishment Report of the organization 6. Applicant/Client supplied
7. One (1) photocopy of document showing Financial Statement of the organization  7. Applicant/Client supplied
8. One (1) photocopy of document showing Profile indicating the purposes and objectives of organization  8. Applicant/Client supplied 
9. One (1) photocopy of document showing Minutes of the Meeting of the organization 9. Applicant/Client supplied
10. For CSO applying to be members of the Local School or Health Board: Photocopy of the profiles of at least three (3) individuals in the organization that will verify their involvement in the health or education sector  10. Applicant/Client supplied 
11. One (1) photocopy of Certification from Tayabas PNP that the group is not involved in any subversive activities 11. Tayabas PNP Station, Brgy. Baguio, Tayabas City
