APPLICATION FOR GSIS SEPARATION/RETIREMENT BENEFIT & PAG IBIG PROVIDENT BENEFIT CLAIM

About the Service: The Human Resource Development Office strictly observes the GSIS and PAG-IBIG policies, rules and regulations to expedite the processing of employees’ retirement benefits

Who May Avail of the Service: Retired and Separated LGU Officials and Employees

Schedule of Availability of Service: Monday to Friday: 8:00 a.m. to 5:00 p.m.


What are the Requirements:

GSIS SEPARATION/RETIREMENT BENEFIT PAG IBIG PROVIDENT BENEFIT CLAIM
1. Duly Accomplished Application form for Retirement/Separation/Life Insurance Benefits (2 copies)

2. Service Record (2 copies)

3. Declaration of Pendancy/ Non-Pendancy of Case (DPNPC) to be submitted upon receipt from GSIS (1 copy original)





 
1. Duly Accomplished Application form for PAG-IBIG Provident Benefit Claim (2 copies)

2. Pag-IBIG Loyalty Card/Loyalty Card Plus or one (1) valid ID of the member (1 Photocopy)

3. Service Record (2 copies)

4. GSIS Clearance

5. PSA Authenticated Copies of Birth Certificate

HOW TO AVAIL OF THE SERVICE:

Applicant to submit accomplished separation/retiremen t form/ accomplished PAG- IBIG Provident Benefit Claim form with complete attachments

Applicant receives the duly signed application form and attachments to be submitted to the GSIS office (optional)



























 
1. Check and receives GSIS application form for separation/retirement/ accomplished PAG-IBIG Provident Benefit Claim and attachments

2. Log the data of the applicant

3. Forward the application for signature of Authorized Official

4. Prepares receiving copy of application form for file copy to be received by the applicant and GSIS/ PAG-IBIG

5. Transmit the application forms and its attachments to the GSIS/PAG_IBIG

6. Advise the applicant to wait for the notification of claim approval from GSIS and PAG-IBIG
