- Photocopy of certificate of eligibility/rating/license.

applications from applicants

or walk-in. a. In case of an applicant person with physical disabilities, his/her application will be received through the assistance of Public Assistance Counter and Complaint Desk (PACCD) situated on the Ground Floor of the City Hall, as deemed necessary. b. For applicants who are speech impaired, hearing impaired and visually impaired, technical assistance will be sought from CSWDO-PWD and SPED of the City Division’s Office of Tayabas and/or other available related institutions. 2. Cross-check of applicants’ documentary requirements. 3. Advise on the status of the application. |
