SCHEDULE OF AVAILABILITY OF SERVICE: Monday to Friday, 8:00 AM to 5:00 PM (No Noon Break)
WHO MAY AVAIL OF THE SERVICE: General Public
DOCUMENTS NEEDED:
• Letter of Request
• Properly Filled-up Freedom of Information Request Form
• Valid Identification Card
PROCESSING TIME:
• For simple request/document – 30 minutes
• For highly technical or complex/sensitive documents: maximum of 3 days
FEES: As enumerated in Chapter VI. Article A. Section 6A.01 and Section 6A.02 of Tax Ordinance No. 18-001 or the Updated Revenue Code of the City of Tayabas in the Province of Quezon.

1 | Proceed to FOI Receiving Officer to begin
transaction. Submit Letter of Request (when available). |

3 | Submit properly filled-up FOI Request Form together with a valid ID. | The FOI Receiving
Officer will review the
submitted FOI
Request Form.
If properly filled-up and the identity of the client has been established based on the valid ID presented, the transaction will proceed with the next step. If improperly filled-up or the identity of the client cannot be established due to invalid ID presented, the FOI Receiving Officer will ask the client to properly fill- up the form and/or present additional valid ID for proper identification. Upon compliance, the transaction will proceed with the next step. |

5 | Wait for the Release of requested information or
document (for simple request).
Will receive advise on the status and date of release of the requested information or document (for highly technical or complex document). |
The FOI Receiving
