FRONTLINE SERVICE: Processing of Business/Mayor’s Permit

ABOUT THE SERVICE: Any person/entity who shall establish, operate or conduct any
business, trade or activity shall first obtain a Mayor’s Permit
and pay the fee therefore and the business tax imposed
under City Ordinance No. 18-001

SCHEDULE OF AVAILABILITY OF SERVICE: Monday to Friday (8:00am – 5:00pm)

WHO MAY AVAIL THE SERVICE: Single Proprietor/Partnership/Corporation/Cooperative

FEES: Payments are based on the Updated Revenue Code of the
City of Tayabas in the Province of Quezon/Tax Ordinance No.
18-001

PROCESSING TIME: New Applicants – 2 Days |
Renewal – 1 Day

DOCUMENTS NEEDED: 
For New Applicants:
1. Duly filled up Application/Unified Form;
2. Proof of business registration, incorporation, or legal personality (i.e.
DTI/SEC/Cooperative Development Authority (CDA) registration;
3. Occupancy Permit;
4. Business Capitalization/Paid-up Capital;
5. Contract of Lease (if Lessee)/Proof of Ownership (if property is owned); and
6. Barangay Business Clearance.

 

Other requirements depending on the line of business:
                  
➤ Zoning Clearance and Building Official Clearance;

                  
➤ Health Clearance/Sanitary Permit;

                  
➤ City ENRO Clearance (for hotel, resort, restaurant, piggery, poultry, hospital,
manufacturing, petroleum and other business establishments that needs City
ENRO Clearance);

                  
➤ For Corporation/Cooperative/Association: Secretary’s Certificate (Authority to
sign/process the business permit accompanied by the valid identification card;

                  
➤ For Single Proprietorship: Photocopy of valid identification card;

                  
➤ For Representative: SPA/Authorization Letter;

                  
➤ For Businesses inside the Public Market: Market Clearance;

                  
➤ If located/inside subdivision: Home Owner’s Association Clearance;

                  
➤ For Pawnshop, Bank or Financial Institution: PNP Clearance;

                  
➤ For Lending Corporation/Investor: Certificate of Authority issued by the Securities
and Exchange Commission;

                  
➤ For Pharmacy/Drugstore and other businesses related to medicine: Affidavit of
Undertaking/Photocopy of PRC License – Pharmacist;

                  
➤ For Repair Shop/Automobile/Electronics/Computer – DTI Accreditation;

                  
➤ For Financing – Authority to operate from SEC;

                  
➤ Fire Safety Inspection Certificate issued by the Bureau of Fire Protection;

                  
➤ For Manpower Agency/Private Employment Agency – License to Operate from
DOLE; and

                  
➤ Others as maybe required by laws/ordinances/circulars.

For Renewal Applicant:

1. Duly filled up Application/Unified Form;
2. Basis for computing tax (i.e. Income Tax Return, Financial Statement, Sworn Statement
of Annual Gross Sales);
3. Barangay Business Clearance; and
4. Other requirements/clearances/licenses/certificates depending on the line of business
being applied.

Other requirements depending on the line of business:

                  
➤ For Authorized Representative: Authorization letter from the business
owner/manager together with his/her valid identification card (photocopy);

                  
➤ Health Clearance/Sanitary Permit;

                  
➤ City ENRO Clearance (for hotel, resort, restaurant, piggery, poultry, hospital,

Clearance);

➤ Licenses/Clearances/Certificates as may be required by the National Laws and
regulatory bodies.
