FRONTLINE SERVICE: Animal Biting Incident and Rabies Investigation

SCHEDULE OF AVAILABILITY OF SERVICE: Monday to Friday, 8am to 5pm

WHO MAY AVAIL THE SERVICE: Any person bitten by pet animal particularly dog or cat

FEES:  P50.00 vaccination and registration fee (applicable for both the biting victim’s pet and the biting animal)
PENALTIES:
As per City Rabies Ordinance 10-01 series 2010
1. Failure to keep their pet inside their properties, enclosed premises and confine in the cage or on leash (sec. 17j)
a. First offense: P200.00 or one day community service
b. Second Offense: P500.00 or two-day community service and confiscation of pet
2. Failure or refusal to have their dog registered and immunized against rabies (sec. 17a)
a. P1,000.00 and their dog shall be seized/impounded by the Animal Control Officer

DOCUMENTS NEEDED:  Community Tax Certificate, Voter’s Certificate and referral slip from the City Health Office (Residence certificate from the barangay if Voter’s Certificate is not available)


HOW TO AVAIL OF THE SERVICE:

A. Secure and Present referral slip from the City Health Office. Community Tax Certificate and Voter’s Registration Certificate from Treasurer’s Office and COMELEC
B. Attend Information and Education Campaign by watching film showing on Rabies Awareness C. Prepare, arrange and setting up of materials such as wide screen, multi-media projector, cd player and sound system 30minutes Janice D. Abracia
D. Sign in the rabies film showing I.E.C. record book
E. Interview / investigate the biting incidence and record details of the biting animal and bitten person 10-20minutes Janice D. Abracia
F. In case the pet is unregistered or unleashed, prepare Registration and Rabies Vaccination Certificate, Citation Ticket and corresponding Official Receipt/s 10minutes Janice D. Abracia/ Christoffer Q. Capil/ Joshua R. Magracia
G. Prepare and fill up referral slip for the City Health Office for the post exposure treatment of the victim to be signed by the City Veterinarian 3-5minutes Janice D. Abracia
