APPLICATION FOR TERMINAL LEAVE BENEFIT
About the Service: Terminal leave benefit is the commutation or conversion of cash of leave credits earned by an employee during his/her employment with the city government. It is given to an employee upon his/her official separation from service via voluntary resignation, retirement, non-renewal of temporary appointment and other mode of severances from the service. An employee has to apply first for this benefit before he/she will claim it.
Who May Avail of the Service: Retired and Separated LGU Officials and Employees
Schedule of Availability of Service: Monday to Friday: 8:00 a.m. to 5:00 p.m.
What are the Requirements:

- GSIS Clearance
- Clearance from money, property and work-related accountabilities
- Leave Card
- Approved Leave Application
- Service record
- SALN
- Photocopy of Notice of Salary Adjustment (NOSA)
- Computation of terminal leave benefits duly signed/certified by the accountant
- Affidavit to deduct all financial obligations with the employer/agency/LGU
Additional Requirements in case of death of claimants:

- Death Certificate authenticated by PSA
- Marriage Contract authenticated by PSA
- Birth Certificates of all surviving legal heirs authenticated by PSA
- Designation of next-of-kin
- Waiver of rights of children 18 years old and above
