SCHEDULE OF AVAILABILITY OF SERVICE: Monday to Friday, 8:00 AM to 5:00 PM (No Noon Break)

WHO MAY AVAIL OF THE SERVICE: Civil Society Organizations, People’s Organization, NGOs


DOCUMENTS NEEDED: 
    a) For CSO Accreditation
           ➤ Duly accomplished Application Form for Accreditation;
           ➤ Board Resolution;
           ➤ Certificate of Registration issued by Registering Government Agency;
           ➤ List of Current Officers and Members;
           ➤ Original Sworn Statement stating that the organization is an independent, non-partisan organization and that our organization will retain its autonomy while pursuing the advancement of the peoples’ interest through its membership in a local special body, after satisfying all the requirements and set criteria, as stated in DILG Memorandum Circular, and after securing a Certificate of Accreditation from the Sangguniang Panlungsod;
           ➤ CY Annual Accomplishment Report;
           ➤ CY Financial Statement;
           ➤ Profile indicating the purposes and objectives of our organization;
           ➤ Copy of the Minutes of the CY Meeting of the organization;
           ➤ Photocopy of profiles of at least three (3) individuals in the organization that will verify their involvement in the health or education sector; and
           ➤ Certification from Tayabas PNP that the group is not involved in any subversive activities.

    b) For other CSO-related concerns and inquiries
           ➤ None (depending on the nature of transaction)

HOW TO AVAIL OF THE SERVICE:

STEP PROCEDURES ACTION OFFICER PERSON IN CHARGE DURATION OF ACTIVITY FORM
1 Proceed to CSO Reception Desk to begin transaction with the CSO Desk Officer.

Submit all Accreditation supporting documents.
CSO Desk Officer will conduct preliminary review of the documents submitted.

Upon determination orderliness of the submitted documents, the CSO Desk Officer will stamp “received” the Application Letter.

The CSO Desk Officer will advise the client to wait for the notification from Sangguniang Panlungsod on the date of claiming the Accreditation Certificate.

If the submitted documents are incomplete or not in order, the CSO Desk Officer will return them to the client with corresponding notes and advise.
