SCHEDULE OF AVAILABILITY OF SERVICE: Monday to Friday, 8am to 5pm
WHO MAY AVAIL THE SERVICE: Any person with healthy, sick or injured pet animals and livestock animals.

HOW TO AVAIL OF THE SERVICE:

| B. The Veterinarian will ask the client the reason/s for consultation, takes medical history of patient, and writes client and animal patient’s data on the veterinary medical record book. | 5minutes | Dr. Isagani C. Requizo | |
| C. Prepares veterinary instruments and medicines available from the Animal Health Center. | 3minutes | Dr. Isagani C. Requizo/ Animal Keeper-I on duty | |
| D. If appropriate veterinary medicines are not available from the Animal Health Center, the City Veterinarian will prescribe veterinary medicine to client and advise to buy the drugs at the nearest veterinary drugstore. | 2minutes | Dr. Isagani Requizo | |
| E. Purchase the prescribed drugs at the nearest drugstore and / or poultry supply store. | |||
| F. Prepares Veterinary Office service vehicle ( if available) | 5minutes | Animal Keeper-I | |

| H. On-site physical examination and restraining of animal patient. The Veterinarian measures vital signs of the animal’s pulse, temperature and respiration; ocular examination, percussion and palpation. | 15minutes | Dr. Isagani C. Requizo/ Animal Keeper-I | |
| I. Treatment of patient – Application of veterinary medicines to cure disease, injuries, or ease symptoms. *Simple case shall be done within 20 minutes *Complex case requires longer period of time to treat the patient | 20 minutes to a day | Dr. Isagani C. Requizo | |
