SCHEDULE OF AVAILABILITY OF SERVICE: Tuesdays & Thursdays 10am to 5pm

WHO MAY AVAIL OF THE SERVICE: Applicants for Certificate of Occupancy Permit


DOCUMENTS NEEDED: 

• Three (3) original copy of Occupancy Permit Forms signed & sealed by Professionals

                    ➤ Unified Application Form for Occupancy Permit (Architect/ Civil Engineer)

                    ➤ Completion Forms of Building, notarized (all professionals – designer & in-charge)

                    ➤ Certificate of Final Electrical Inspection (PEE/ REE/ RME)

                    ➤ Completion Forms of Sanitary (SE/ RMP)

                    ➤ Completion Forms of Mechanical, if applicable (PME)

                    ➤ Completion Forms of Electronics, if applicable (PECE)

• One (1) original copy Fire Safety Inspection Certificate

• One (1) Original copy Final Zoning Compliance

• One (1) original Construction Logbook signed & sealed

OTHER REQUIREMENTS:

• Printed Picture of the Structure & Installed Permanent Meter base

• Testing Results & PME Certification for Systems/ Equipment (i.e. Steam Boiler, Pressure Vessel, Elevator, Escalator, and the likes)

• Installed Earthquake Recording Instrument

• Location Sketch to the nearest landmark

• Copy of Issued Permit Forms

• PTR/PRC of Professionals

• Valid IDs

• Affidavit of Change of Engineers/ Waiver (if applicable)

PROCESSING OF DOCUMENTS: Every day except weekends & Holidays 8am to 5pm

FEES: Based on the National Building Code of the Philippines PD 1096
