About the Service: Certifications are the common documents being requested by LGU Officials and Employees. The documents are usually attached in claims for GSIS and PAG-IBIG. It is also a requirement in Loan application.

Schedule of Availability of Service: Monday to Friday: 8:00 a.m. to 5:00 p.m.
HOW TO AVAIL OF THE SERVICE:

||Fill out and submit Freedom of Information (FOI) form for the request of Service Record and/or Certificate of Employment.
||1. HRMO Staff- System Administrator of PMIS will receive the Freedom of Information form
2. Retrieve from computer database employee’s service record data/COE

|Freedom of Information Form Customer Service Feedback Form;
Form 1-‘end of transaction’ feedback form available in the offices
|3. Prepare and Print Service Record/ Certificate of Employment
4. Head of HRMO will review and sign the service record/certificate of employment
|Ms. Aira R. Tabernilla- Administrative Assistant II (HRMA)
||Form 2- ‘Random survey’ Walk-in complaint form available in PACD
|5. Release Service Record and/or Certificate of Employment to the requestee|
