SCHEDULE OF AVAILABILITY OF SERVICE: Every day except weekends & holidays. 10am to 5pm

WHO MAY AVAIL OF THE SERVICE: Applicants for Optics, Pole Installation, and the likes


DOCUMENTS NEEDED: 

• One (1) original copy of Brgy. Clearance/s for Excavation

• Two (2) Gantt chart signed & sealed by Project Engineer

• Five (5) original copies of Excavation Permit signed & sealed by Civil Engineer

• Five (5) plans signed & sealed

• One (1) Original copy Locational Clearance

• One (1) Original copy DPWH Clearance/ PEO (if applicable)

• Five (5) original copies Technical Documents signed & sealed

OTHER REQUIREMENTS: 

• PTR/PRC of Professionals

• Secretary’s Certificate (if needed)

• Memorandum of Agreement between MERALCO & Applicant/ MOA (if applicable)

• Valid IDs

PROCESSING OF DOCUMENTS: Every day except weekends & Holidays 8am to 5pm

FEES: Based on the City Tax Ordinance No. 18-001/ National Building Code of the Philippines PD 1096
