Checklist from PSA

– Tutungo ang kliyente sa Clientele assistance Desk dala ang filled-out request form

– Assessment ng mga requirements. Ang kliyente ay bibigyan ng color coded number card kalakip ang Clientele feedback form.

– Isusulat ng kliyente ang kanyang mga impormasyon sa left lower box ng Clientele Assistance Form (Pangalan, Address at Contact Number)

– Tatawagin ang numero ng kliyente at ibibigay sa nakatalagang empleyado ang clientele assistance form at mga dokumento.

Pagpi-presenta ng dokumento:

– Ibibigay ng kliyente ang mga dokumento (current year issue) para sa assessment.

– Maghihintay ang kliyente habang ina-assess ang mga dokumento

– Magbabayad sa Payment Window ng CCR para sa itinalagang bayarin

– Ihahanda ng itinilagang empleyado ang mga dokumento para sa endorsement at ipapasa para sa final review ng dokumento bago lagdaan ng Tagatalang Sibil.

– Sa nalolooban ng limang araw, ipapadala sa PSA Lucena ang dokumento at mga kalakip nito para sa proseso ng annotation ng birth certificate, kasabay ng iba pang dokumento.

– Isang araw matapos iendorse ang dokumento sa PSA Lucena ay ipagbibigay-alam sa kliyente ang skedyul na ibinigay ng PSA para sa follow up.

Lalagda ang kliyente sa logbook para sa releasing ng dokumento at stub.
