1 Submit document/s (Request Letter; Applications for Leave of Absence or Travel Order except for Elected Official and other incoming correspondence) Receives and Records submitted document or communication Administrative Staff 3minutes Customer Service Feedback Form; Form 1- ‘end of transaction’ feedback form available in the offices Form 2- ‘random/walk -in complaint form available in PACD.
2 Follow-up Reviews and approves the document and gives recommendations for proper action.

Releases/ Endorses signed document to the concerned department office/ agencies/ personnel for appropriate action.

Informs the requesting party the status of the document
