FRONTLINE SERVICE: Processing of Motorized Tricycle Operator’s Permit (MTOP)

ABOUT THE SERVICE: The Motorized Tricycle Operator’s Permit (MTOP) is a document granting franchise or license to a person, natural or juridical before they can operate a tricycle-for-hire in the City of Tayabas.

SERVICES: 
a. Application Form Issuance
b. MTOP Issuance
c. ID Issuance
d. Fare Matrix Issuance
e. Dropping Form Issuance
f. Certificate of Dropping Issuance

SCHEDULE OF AVAILABILITY OF SERVICE: Monday to Friday (8:00am – 5:00pm)

WHO MAY AVAIL THE SERVICE: Only Filipino citizen and partnership or corporation with sixty percent (60%) Filipino equity under City Ordinance No. 18-001 and must be a registered voter of the City of Tayabas as per E.O. No. 03, series of 2015

FEES: Payments are based on the Updated Revenue Code of the City of Tayabas in the Province of Quezon/Tax Ordinance No. 18-001

PROCESSING TIME: New Applicant/Renewal – One (1) Day

DOCUMENTS NEEDED: 
For New Applicants:
a. For Brand New Unit
         ➤ Photocopy of Motorcycle/Tricycle Certificate of Registration (C.R.) and latest Official Receipt (O.R.) issued by the Land Transportation Office (LTO);
         ➤ Photocopy of valid identification card;
         ➤ Photocopy of community tax certificate (cedula); and
         ➤ Photocopy of latest COMELEC certification.

Note: In the absence of the registered owner, the authorized representative must present a Special Power of Attorney or a notarized letter from the owner together with his/her valid ID’s.
If the registered owner is working abroad, the representative will present his/her community tax certificate.

b. Additional requirements if “Second Hand” unit bought from Casa
           ➤ Original and photocopy of Motorcycle/Tricycle Certificate of Registration (C.R.) and latest Official Receipt (O.R.) issued by LTO;
           ➤ Original and photocopy of the surrendered paper of the said motorcycle from the previous registered owner;
           ➤ Original and photocopy of the Deed of Sale executed by and between the buyer/applicant and Lending Institution who will finance the purchase and where the motorcycle will be mortgaged. In such case, the photocopy of the Certificate of Registration issued by the LTO and duly certified by the lending institutions shall be submitted by the applicant instead of the original copy; and
           ➤ Original and photocopy of the Corporate Secretary’s Certificate (in case of corporation) or Special Power of Attorney (in case of individual) as to the authority of the signatories on the Deed of Sale.

c. Additional requirements for “Second Hand” unit bought/acquired from any individual and that same is free from any encumbrance and/or obligation
         ➤ Original and photocopy of Motorcycle/Tricycle Certificate of Registration (C.R.) and latest Official Receipt (O.R.) issued by LTO;
         ➤ Original and photocopy of the duly notarized Deed of Sale executed by and between the buyer/applicant and the previous registered owner;
         ➤ Original and photocopy of the Deed of Sale executed by and between the buyer/applicant and Lending Institution who will finance the purchase and where the motorcycle will be mortgaged. In such a case, the photocopy of the Certificate of Registration issued by the LTO and duly certified by the lending institutions shall be submitted by the applicant instead of the original copy;
         ➤ Original and photocopy of the Corporate Secretary’s Certificate (in case of corporation) or Special Power of Attorney (in case of individual) as to the authority of the signatories on the Deed of Sale;
         ➤ Valid identification card;
         ➤ Community Tax Certificate (cedula); and
         ➤ COMELEC Certification.

For Renewal:
Requirements:
      ➤ Photocopy of Motorcycle/Tricycle Certificate of Registration (C.R.) and latest Official Receipt (O.R.) issued by the Land Transportation Office (LTO);
      ➤ Photocopy of Valid Identification Card;
      ➤ Photocopy of Community Tax Certificate (cedula); and
      ➤ Motorized Tricycle Operator’s Permit (MTOP) of the preceding year.

Note: In the absence of the registered owner, the authorized representative must present an authorization letter from the owner signed by him/her together with his/her valid ID’s.
If the registered owner is working abroad, the representative will present his/her community tax certificate.

Procedures/Steps:
1. Bring and present all the requirements at the Tricycle Franchise Section/BPLO for checking and verification to secure an application form;
2. Read and understand thoroughly the content of the application form before affixing the signature and have it signed by the Barangay Chairman and the Traffic Management Officer;
3. Proceed to the Tourism Administrative Section for the orientation schedule;
4. Have your application form notarized of the Notary Public Officer;
5. Proceed to the Office of the City Treasurer for payment of the franchise fee;
6. Back to Tricycle Franchise Section/BPLO for the issuance of Motorized Tricycle Operator’s Permit (MTOP);
7. Proceed to the Office of the City Treasurer for the signature of the Chairman of the TTFRB.

Dropping/Cancellation of Tricycle Franchise
Requirements:
       ➤ Issued Motorized Tricycle Operator’s Permit (MTOP) at the City of Tayabas;
       ➤ Photocopy of Motorcycle/Tricycle Certificate of Registration (C.R.) and Official Receipt (O.R.) issued by the Land Transportation Office (LTO);
       ➤ Photocopy of Valid Identification Card;
       ➤ Photocopy of Community Tax Certificate (cedula);
       ➤ Franchise Plate.

Note: If acquired through a deed of sale/manahan and the tricycle franchise was not surrendered by the previous owner, he/she can apply for the cancellation of franchise provided that he/she can submit the above-listed requirements (original and photocopy) together with the original and photocopy of the deed of sale/manahan document.

Procedures/Steps:
1. Bring and present all the requirements at the Tricycle Franchise Section/BPLO for checking and verification to secure an application for cancellation of franchise.
2. Read and understand thoroughly the content of the application form before affixing your signature and have it signed/approved by any three (3) of the following: Chief of Police, Traffic Management Officer, Accredited Tayabas NGO’s or SP Member, Chairperson of the Committee on Franchises, Transportation, Communication, Parking and Utilities;
3. Have the application for cancellation notarized.
4. Proceed to the Office of the City Treasurer for payment of the cancellation/dropping fee.
5. Then, proceed to Tricycle Franchise Section/BPLO to secure a Dropping Certificate.
