SCHEDULE OF AVAILABILITY OF SERVICE: Mondays – Fridays 8am to 5pm

WHO MAY AVAIL OF THE SERVICE: Clients who need permits


DOCUMENTS NEEDED: Request letter, Names and Personal Information of the clients (Name, Age, Address, Sex)

FEES: PhP 50.00/climber for the environmental and entrance fee


HOW TO AVAIL OF THE SERVICE:

STEP PROCEDURES ACTION OFFICER PERSON IN CHARGE DURATION OF ACTIVITY FORM
1 Proceed to the City ENRO. Request permit to climb in Mt. Banahaw de Tayabas through letter or verbally. Receive letter of request to climb if there any.

Interview the client/s and get the personal information of the climbers (name, address, sex, age) as per required by the DENR

Orient the client on the rules and regulations when inside the protected area of Mt. Banahaw

Form 2- ‘random/walk -in complaint form available in PACD.

Permit to climb
2 Wait for the permit Get contact number of the client to inform them if their application is approved and when to get the permit (normally the release of permit will take 1-2 weeks)

Send the request letter and personal information of the climbers to the DENR-PAMB through e-mail

Inform the applicant if their application is approved and when they can get the permit through mobile
3 Proceed to the City ENRO; pay for the entrance fee and get the tickets; receives the permit to climb Issues the permit and entrance fee tickets imposed by the DENR

Record the ticket control number to the record book

Let the client sign the receiving copy as proof that the permit is already released
