SCHEDULE OF AVAILABILITY OF SERVICE: Monday to Friday (8:00 AM to 5:00 PM)
WHO MAY AVAIL OF THE SERVICE: The General Public – Students, Researchers, Private Investors, Government Offices, and anyone who needs basic data of Tayabas.
DOCUMENTS NEEDED: Letter of Request Addressed to: Gino Carlo C. Zaporteza, OIC-OCPDC; or Fill-up the Client’s Inquiry and Research Slip
PROCESSING TIME: 20minutes
HOW TO AVAIL OF THE SERVICE:

|1||Proceed to the front desk and pre- sent the Letter of Request or ask for Client’s Inquiry and Research slip.||Gives the Client’s Inquiry and Research Slip (CIRS) to the Client and allow to fill it up.||Front Desk Staff||1minute||Client’s Inquiry
Customer Service Feedback Form: Form 1 – ‘end of transaction’ feedback form available in the offices
Form 2 – ‘random survey’/walk-in complaint form available in PACD
Fill-up Public Assistance form and received the data needed.
|2||Fill-up the Slip and Indicate the re- quested data.||Explain to the client the Output needed if it is printed, Photocopy or in CD and the necessary fees indicated in the Schedule of Fees under Revenue Code.||Front Desk Staff||2minutes|
